Click Submit a request.
From the drop-down list, select Health Harmony.
For a question about Health Harmony, enter the following required information:
- Description of the request. Please be as specific as possible.
- Patient ID or medical record number. This is a secure field.
- CC – Enter one or more email addresses.
- Organization – If you belong to more than one organization, you’ll see an Organization field. Select which Organization relates to your request.
- Attachments – Add a relevant file, possibly a screenshot, up to 20 MB in size.
